These forms are to be completed by those mentoring the student.
The Life and Ministry Assessment evaluates the student on the basis of the qualification and character lists in the Pauline Epistles. It is designed to identify areas of solid and needed growth in the life of the student.
The Becoming Established Assessment evaluates how well the student is established in Christ's teaching, on the basis of Paul's Early, Middle, and Later Letters. It is designed to identify areas of solid and needed growth in becoming fully established.
The Ministry Team Profile Assessment evaluates the student based on completed ministry assignments, his/her SIMA profile, Giftedness Assessment, and recommends a role for him/her on a ministry team.
The Current Ministry Assessment evaluates the student on the basis of his/her current ministry assignments and the clear areas of giftedness and needed development that are manifesed in that ministry assignment.
The Journal of Mentor Assessment is a journal by the student's mentor discussing his experience with the student for the duration of the program.
The Giftedness Assessment is designed to help the student identify his/her gifts based on the gift lists of Paul and Peter. It will aid in identifying good ministry fits that balance ministry needs and the giftedness of the individual as well as sharpening ministry development goals.
The SIMA Autobiographical Form collects data from the student about achievement activities that he believes he did well, enjoyed doing, and/or had a personal sense of satisfaction in doing. The data will then be used to produce a highly personalized Motivated Abilities Pattern (MAP) for the student.
The SIMA CAP Autobiographical Form collects data from the student about achievement activities that he believes he did well, enjoyed doing, and/or had a personal sense of satisfaction in doing. The data will then be used to produce a Computer Assisted Pattern (CAP) for the student.
